The CCC Plus Waiver formerly known as (EDCD waiver), requires people to meet both financial and functional criteria. For financial eligibility, only the income/assets of the person seeking Medicaid are considered. Children are considered a household of one for the purposes of the CCC Plus Waiver financial determinations. Functional eligibility is determined through a screening process completed by the person’s local Department of Health (DOH). In order to be approved, you must meet the eligibility criteria for both financial need and functional dependencies. Skilled Nursing

Harvestmen Home Healthcare Agency skilled nursing services requires a medical provider order and is approved through Medicaid. This service of nursing is individual-specific and may take place based on the person’s care needs. It is supported by a customized nursing care plan which is individualized and person-centered, and a part of the customized Plan for Care/Support. Skilled Nursing services at Harvestmen Home Healthcare Agency are specific to an individual’s clinical, daily, and overall functional needs.

Daily Care

Harvestmen Home Healthcare Agency offers non-medical such as bathing, toileting, meal preparation and skilled medical care such as wound care to individuals of all ages who are bound at home. Being homebound means a variety of things such as fragile health, physical disability, mental challenges and transition to old age. We care, so let us help.

Every individual will have a care plan that is customized to fit his or her needs. The care plan for each individual will include a variety of Class A services that we offer. With your customized plan of care, our leaders and caregivers will make it our priority that your overall needs are addressed decently and in order.

Private Duty Nursing

Harvestmen Home Healthcare Agency Private Duty Nursing can be used to support individuals with intensive care needs and is supported with a nursing care plan which addresses the care tasks essential to a person’s health and safety. This level of nursing also requires a medical provider order for Medicaid approval and is supervised by Harvestmen Home Healthcare Director of Nursing Supervisor.
