These waivers provide access to comprehensive services and support to individuals living with developmental and/or Intellectual developmental disabilities. Harvestmen Home Healthcare Agency offers the following services; Skilled Nursing, Private Duty Nursing, and In-Home Support Services (IHSS) for those who receive CL/FIS waiver. Harvestmen Home Healthcare Agency provides In-Home Support Services (IHSS) to individuals ages 3 to 90+ with a diagnosis of intellectual disabilities, mental illness, and/or other developmental disability. Here at Harvestmen Home Healthcare Agency we put the needs of the individuals we support first. Harvestmen Home Healthcare Agency directors and staff have 10+ years of experience in catering to the needs of those with intellectual disabilities, Our staff understand the importance of using team-based approaches and person-centered practices to fully support those who have significant support needs.
Skilled Nursing
Harvestmen Home Healthcare Agency skilled nursing services requires a medical provider order and is approved through Medicaid. This service of nursing is individual-specific and may take place based on the person’s care needs. It is supported by a customized nursing care plan which is individualized and person-centered, and a part of the customized Plan for Care/Support. Skilled Nursing services at Harvestmen Home Healthcare Agency are specific to an individual’s clinical, daily, and overall functional needs.
Private Duty Nursing
Harvestmen Home Healthcare Agency Private Duty Nursing can be used to support individuals with intensive care needs and is supported with a nursing care plan which addresses the care tasks essential to a person’s health and safety. This level of nursing also requires a medical provider order for Medicaid approval and is supervised by Harvestmen Home Healthcare Director of Nursing Supervisor.
In-Home Support Services
Harvestmen Home Healthcare Agency has a customized program that provides support to individuals who reside in their own home. Individuals may live by themselves or have their natural support which will consist of friends and family. Our (IHSS) program focuses on the family's strengths and needs. Our team of professionals will conduct an assessment to determine how the person can best be supported to live as independently as possible, with the maximum quality of life within their own home and community. Harvestmen Home Healthcare Agency requires Individuals that live on their own to be able to function independently most of the time, able to identify and avoid danger, as well as to know when and how to call for help. Harvestmen Home Healthcare Agency (IHSS) program provides services for individuals and families that are willing to assist in the directing of the service for the individual who is being served. This will include hiring, supervising, and scheduling of caregiver staff along with assisting with a customized plan of care to better support the individual. Utilizing Harvestmen Home Healthcare Agency (IHSS) programs focus on the
compatibility and safety of others already living in the environment. Our agency has Qualified Mental Health Professionals who provide customized training for our staff on the needs of the individual in which we serve. Our agency also helps staff to understand the nature of mental illness along with having customized supervision that is needed for our individuals in order to support our individuals with a better quality of living
